NHS Devon CCG commission community stroke support services across Devon\. Within West Devon there is also an enhanced service to provide 6-week, 6 month and 12 month post stroke review check within Plymouth footprint in the community\. Devon CCG has a budget of £256,863 to deliver these services in conjunction with Plymouth City Council \(for the Western and Plymouth element\) for a period of 2 years \(01/04/2020-31/03/2022\)
