Work description

Provision of peripatetic nursing services in Kensington, Chelsea and North Westminster for 6 months.

The aim of the service is to reduce health inequalities and improve health outcomes for the homeless residents placed within the local hostel accommodation.

The peripatetic nursing service will: - work closely alongside hostel staff and meet with clients to complete health and wellbeing assessments on-site. - provide follow-up review to meet the client's health and well-being needs and ensure referral and facilitation into primary care and specialist services. - target homeless drop in day centres and churches within the borough. - work within a motivational framework to assist clients to think about adopting healthier behaviours as well as supporting clients to address often serious and frequently untreated health problems. - have developed enhanced referral pathways into services to maximise uptake and provide much needed screening/brief intervention and follow-up support. - provide ad hoc training and support to hostel staff to assist them in managing residents with complex on-going health needs.

Additional information: This is an interim contract for only 6 months to enable the newly formed CCG (North West London CCG) from 1st April 2021 to review its commissioning strategy for this and related community based services.
