## Description

Provider Selection Regime - Direct Award Process C - \(PSR\) \\r \\r Provision of Halton 0-19 \(\+25 SEND\) Healthy Child Programme, Public Health Service Contract\.\\r \\r The Halton 0-19 \(\+25 SEND\) Healthy Child Programme, integrated Public Health Nursing Service will provide clinical leadership, co-ordination, and delivery of the Healthy Child Programme 0-19 \(\+25\) HCP in the Halton Borough Council\. The service will be led by health visitors and school nurses working in partnership with a range of professionals and agencies to support children and families and to deliver the Healthy Child Programme\. Health visitors and School Nurses will be specialist community public health nurse \(SCPHN\); they will be registered nurses or midwives who has undertaken a year's further post registration training in child health, health promotion, public health, and education\. Health Visitors and School Nurses will provide place-based services and work in partnership with education, LAs, health and other key partners where needed to deliver an effective Healthy Child programme for prevention and support\. The service will work alongside other child and family services who will be delivering other Public Health elements of the Healthy Child Programme\. As an essential part of the children's services 'system', the Service will make a major contribution to the Halton Borough Council Children and Young People's Improvement Plan ambitions by improving health and wellbeing outcomes for children and young people, identifying additional needs early, building resilience and reducing health inequalities by providing effective universal and targeted interventions for children, young people and their parents\.
