Immunisation E-Consent System

SHFT operates across all types of schools (private, independent and state) in Hampshire delivering immunisations to approximately 150,000 children aged 4-16 years old. The size of immunisation programme is increasing and we are looking for a safe and paper-light process to provide an e-consent programme. We need a system that will - be accessible to all users through schools and Hampshire County Council websites; and all SHFT immunisation service - be able to filter the consent forms to those that give consent and those that don't. - highlight forms with concerns - highlight forms that require a translation facility - have robust security protection to ensure all IG and GDPR is observed - link with SHFT Electronic Patient Record - be able to work off-line if necessary - be available on mobile devices SHFT need the ability to work with the provider in the construction of the consent form itself to ensure that the correct information and data is asked for and collected. Cohorts change during the year and the database will also need to be updated during the course of each academic year.
