## Description

The Council was seeking to appoint a provider to deliver a Community Discharge to Assess Service to support timely discharges from hospital settings\. This was a call off from the 'Pseudo Frameworks for Spot Provision of Home Care Services' and only Providers currently approved on the frameworks could apply\. Providers approved on any of the three Zones could apply however, only one provider to be appointed to cover this service across all Zones\. Therefore, Providers could only submit a bid if they could operate across all zones to deliver the service in its entirety
