Description
This project is in the Service Area described as Working Age Adults (WAA, under 65) and specifically where Nottinghamshire County Council (NCC) meets the needs of individuals via an admission to a CQC registered Residential Care Home and/or a Nursing Home. This contract is a collaborative approach between the Provider and NCC to: a. undertake a rapid review of all current WAA residential and nursing placements. b. jointly identify a cohort of placements where we agree that our approach to assessment, review and commissioning could alter to deliver efficiencies and better outcomes. c. support NCC to begin to implement an improved, more cost effective and clear strengths-based approach to our assessment, review and commissioning of individuals who are currently in residential and nursing care and those who may enter these services in the future.
