## Description

The Brokerage Service is a bespoke intervention to facilitate prompt admissions into a care home placement where an individual requires long term residential / nursing care from the community\. The Brokerage Service will support Service Users who are assessed by the Council as needing long term residential / nursing care and referred to the Service for support in identifying a suitable care home that meets assessed needs\.
