NHS Dorset ICB is developing the business requirements for a Remote Monitoring Partner solution to enable professionals to have the ability to collect and then observe information about a patient whilst the patient is not in a clinical setting (in a place they call home, including care homes). In scope is long term conditions remote management (LTCRM), early deterioration and remote monitoring (RM).
