## Description

Provision of ‘Help Not Hospital’ Service to support Havering's vulnerable adults as they are discharged from hospital back into the community\. This service forms a part of the preventative offer for Havering residents, providing support for people to regain their independence after a stay in hospital\. Additional information: The contract may be extended for up to twelve months beyond the initial termination date of 30 06 19\.
