Work description
The CCG, on behalf of the Place Based Partnership (PBP), is looking for a partner to support the Leeds Place Based Partnership PBP to strengthen a range of Population Health Planning capabilities through the review (and where necessary refining) of the Leeds Population Segmentation Model and Outcomes Framework Development.
