Older Persons' Prevention & Support Service

All documentation is available at www.yortender.co.uk. The Council's intension is to commission a single provider to deliver a Prevention and Support service for Older People within Calderdale. The purpose of this service is to provide a mixed provision of support to people who are over the age of 60 and who need support to sustain independent lives in their own home. The focus of the Service shall be on supporting clients to maintain accommodation and provide on-going support to enable people to effectively manage and sustain their housing needs, with the aim of improving their ability to maintain their independence and avoid deterioration in health which results in admission to hospital or a residential care home.
