Work description
The service contract with West London CCG (WLCCG), now NWL CCG, is for funding the At Home Respite for carers of people live with Dementia. The service is run by Age UK K&C. Key elements of the service include: • Offering a break for carers giving the cared for person a safe and supportive environment • Supporting carers by enabling them to have a respite break from caring, so as to continue in their informal caring role • Providing a safe and welcoming atmosphere for emotional support, social interaction and information • Involvement of volunteers and health care professionals
Additional information: A new contract is putting in place for a period of 18 months to afford the newly forming NHS North West London CCG time to agree the approach to commission this service at NWL ICS level in line with national and ICS guidance to ensure smoother, less complicated, non-duplicated service delivery.
