## Description

This contract is for Homeless Support Service for Young People \(including care leavers or looked after children\)\. Durham County Council recognises the need for a holistic support model that will help to create a pathway of provision for young single males and females who are at risk of or experiencing homelessness\. A person-centred approach is needed for those who would struggle to access accommodation without the help and assistance from support services\. The Council is wishes to commission a support service to enable the Council to meet its statutory duties and respond to the accommodation needs of local young people aged 16-17 years, including Looked After Young People and Care Leavers \(up to age 21years\)\. The Service will provide low to medium levels of support\.
