## Description

The aim of the service is to provide assertive outreach to rough sleepers\. In addition to support and accommodation the outreach service would be responsible to carry out an early morning sweep twice a week, this would be linked in with other services such as Police and Housing Solutions staff\. Service staff will maintain daily contract with rough sleepers and will consider health and wellbeing as well as signposting to other agencies and support services\.
