Work description
Durham County Council holds the budget for the regional Integrated Care System (ICS) Prevention and Population Health Management Board, which is to be used to fund the NENC ICS Healthier and Fairer Programme Support. This includes the employment of a Population Health Management (PHM) team (1wte Band 8b Senior Programme Manager, 0.5wte Band 8a Programme Manager, 1wte Band 7 Project Manager plus analytical support) which will develop tailored PHM programmes at ICS workstream level.
