Work description
This is a notice for social and specific services in accordance with Directive 2014/24/EU Article 74?
The Council wishes to commission a cost effective Memory Support Service to support people living with dementia in Buckinghamshire. The aim is to provide a range of support services to people living with memory concerns, dementia and their carers.
The Service will support the health, wellbeing and independence of Service Users in a community setting and be delivered on behalf of Buckinghamshire County Council (the Council), in partnership with Aylesbury Vale Clinical Commissioning Group (AVCCG) and Chiltern Clinical Commissioning Group (CCCG).
The budget for this service is £156,000 per annum.
The contract will be awarded initially for 3 years with a possibility to extend for a further two years, depending on performance and availability of funding.
Transfer of Undertakings (Protection of Employment) Regulations 2006 (TUPE) may apply to this contract.
