Specialist Vision Assessment & Rehabilitation Support Service

The overall aim of the Service is to support Hartlepool's adult population with specialist vision assessments and deliver vison rehabilitation.

The following service outcomes have been identified : - Facilitate the completion of specialist vision assessments - Where necessary refer and deliver specialist rehabilitation in a timely manner - Support people to maximise their independence - Support people to maximise their quality of life
