Work description
Provision of an Integrated Care Centre Service for patients in Herne Bay. The overall aim of the service is for the provision of an Integrated Care Centre for patients from the Herne Bay area to provide:
- a locally accessible integrated model to include access for minor injury/illness including x-ray diagnostics, enabling easy and timely access to urgent health services, ensuring patient safety - access to a range of community based services to include DVT, catheter care, wound clinic, musculo-skeletal services, and frailty clinic.
The contract will be for an initial three year term, with an optional two year extension. The contract value covers the whole five year term. The opportunity is further described in the Memorandum of Information MOI and the Service Specification attached to this notice.
The Commissioner is NHS Canterbury and Coastal CCG. The Commissioner is supported in this project by NHS South East CSU (SECSU) and its procurement partner NHS Commercial Solutions (NHSCS), which is managing this procurement.
