Work description
Durham County Council holds the budget for the regional Integrated Care System (ICS) Population Health and Prevention Board, which is to be used to fund Population Health Management (PHM) Support to the North East and North Cumbria, which has an emphasis on PHM approaches to reduce health inequalities across the area. This is for the period of 1 September 2022 - 31 March 2023 The provider of this service is North of England Commissioning Support Unit (NECS)
