Community Socially Prescribing Service Empowering and supporting people to establish new relationships, social opportunities and support networks within their community in order to reduce social isolation; loneliness and improve their health and wellbeing\. The social prescribing approach aims to break social isolation and promote social connectivity through facilitating access to locally based activities which both directly and indirectly improve health, boost confidence and provide a sense of community\.
