Work description
Provision of an end to end care market management solution. Call-Off via Crown Commercial Services (CCS) G-Cloud 12.
In May 2025, LCC modified this contract to include the implementation of an electronic care monitoring, management and scheduling module for Intermediate Care Services. The modification does not change the overall nature of the original contract and is not substantial.
