Work description
Pilot contract to deliver a high quality Care Act assessment service for Sandwell Adult Social Care. The assessments will consist of: • Assessments of Carers • Assessments or reassessments of adults with needs for care and support • Review of packages of care and support at 4 weeks or sooner if required
