## Description

This Procurement is for a new Tier 2 Community Dermatology Service in Blackpool, Fylde and Wyre\. The main outcomes of the Service will be: • Equitable access and treatment of dermatology patients presenting with dermatological problems within different levels of deprivation\. • Increased overall patient's satisfaction with Dermatology Services\. • Provider services closer to patients' home\. • Improved communication between community, secondary and primary care services provider, specialist clinicians and Primary care\. • Improved quality of care within primary and community settings regardless of provider\. • Increased patient choice\. • Improved access to advice and information, increased knowledge and awareness of the management of dermatology for patients\. • Reduced waiting time for patient's access to specialist care in the community\. • Improve access to advice and information with increased knowledge and awareness of the self-management of their condition\. The contract duration will be for 3 years, which is intended to run from 1st of November 2018 until 31st October 2021 with the possibility of one extension of up to 2 years \(31st October 2023\)\. The anticipated, maximum total contract value \(including any potential extensions\) is £3,605,441 with an annual value estimated at £ 721,088\.
