Work description
The programme aims to identify opportunities to improve collaboration between system partners to enhance planning and implementing hospital discharge, recovery and reablement in the community. The procurement of an experienced delivery partner to support the work of the programme, though expert knowledge and additional transformational resources, is considered to be key in achieving these ambitions.
