A physical activity programme for Somerset which integrates and co-ordinates existing health walk provision and a new and updated exercise referral programme for Somerset to support improvements in physical activity levels to deliver population health improvements for the most disadvantaged people with long term conditions where physical activity is evidenced to improve self-management\.
