## Description

The Tri-borough Hospital to Home Discharge Support service works alongside health &amp; social care partners to support the discharge of Tri-borough patients from Imperial and Chelsea and Westminster Hospital\. The Hospital to Home service delivers a service that; • Streamlines the discharge process including supporting Discharge to Assess pathways • Provides an integrated service between Health and Social Care, operating both in the hospital prior to and during discharge, as well as in the community • Improves outcomes and ensures vulnerable older people settle at home and maximise their independence • Empowers individuals to maximise choice and control over their life and promote health &amp; well-being • Enables people to continue to live in their own home for longer and to avoid or delay the need for costly support services and packages of care later • Enables access to other community services\. Additional information: Direct award of a contract for a period of 1 year \+ 1 year to enable the CCG to allow the ICP and system partners the time and opportunity to conduct a full review of future service requirements and operating models\. Recognising its obligations and responsibilities under the NHS \(Procurement Patient Choice and Competition\)\(No\.2\) Regulations 2013, the CCG has assessed the most appropriate route to awarding this contract, having regard to the necessity to secure continuity of these essential patient services and has concluded that the incumbent provider will assure the safest solution and being considered the only technically capable provider at this time\.
