Edge of Care - CPG 843-W

Description

A bespoke multi-disciplinary virtual team comprising social work practitioners, family support professionals and others specialists to work with a cohort of 50 children and young people who have been identified as being on the 'edge of care'. The team will work intensively with the cohort over a 6-month period with the specific aim of keeping them out of care. There is a Risk and Reward element to the contract which is based on the sustainability of the identified cohort remaining out of care following the intervention. Direct award to Skylakes.
