This Contract is for the provision of an Embedded Mental Health Worker Service as part of the Oxfordshire Out of Hospital Care Model\. The Embedded Mental Health Workers \(EMHW\) will operate within housing and hospital systems across Oxfordshire to identify and support people experiencing a deterioration in their mental health, with the overall aim being to avoid rough sleeping and homelessness and unnecessary \(re\)admissions to hospital settings where a person's needs may be better met in the community\.
