Pan-London NHS CHC domiciliary care AQP - phase 3

Description

Phase 3 of the pan-London NHS CHC domiciliary care AQP contract is open for applications from 30 April 2018 to 8 June 2018 at https://procontract.due-north.com. Successful providers will be added to a domiciliary care AQP list which is used by London Clinical Commissioning Groups (CCGs) and discharge teams to make continuing healthcare domiciliary care placements. The AQP is the standard route of commissioning for the participating CCGs. The contract will use the full-length NHSE Standard Contract.
