Community Medical Support

Description

Solent NHS Trust ('the Trust) is inviting quotations from suitably qualified providers to deliver 'Medical support for Solent's Intermediate Care Plan' 2. Background information 2.1. The focus of this requirement is to support the specialist community services Solent NHS Trust provides to adults with frailty or who are pre-frail. Solent NHS Trust provides a broad range of services to this patient group which includes, but is not limited to:- - Proactive prevention services working closely with Portsmouth City Council and the non-statutory sector. - Inpatient, outpatient and home-based specialist rehabilitation and re-ablement services. - Crisis support - Discharge to assess support services - Condition/Need-specific support services including specialist community heart failure, respiratory, palliative care and neurological management. - End of life care 2.2. Medical Support is required to two inpatient wards (Spinnaker and Jubilee) and PRRT and their dedicated practitioner through routine ward rounds and telephone ad hoc advice during contracted hours as set in service specification document.
