LOT 2: Integrated Adult Weight Management and Exercise on Referral - the requirement The aim of the Exercise and Weight Management Service is to improve the wellbeing of adults in Camden and Islington by delivering sustainable increases in physical activity and reduced overweight and obesity to eligible populations. There will be two streams to this service: ? a) The Exercise on Referral stream will increase the physical activity levels of population groups with specified health conditions in Camden and Islington to support the prevention and management of particular health conditions. ? b) The Weight Management stream will deliver a community based tier 2 weight management service for overweight adults (body mass index ?25 to ?35kg/m2) with comorbidities and obese adults (body mass index ?30 to 40kg/m2) in Camden and Islington. The service should be easy to access and should improve service users? skills, knowledge and confidence in healthy eating and physical activity, resulting in long term maintenance of physical activity and weight loss.
