NHS Fylde and Wyre Clinical Commissioning Group (CCG) serves a population of approximately 151,000 people, registered with 20 GP practices. End of Life Care is a key workstream for the CCG, identified in our, “2030 Vision for Health and Care in Fylde,” and, “Fylde and Wyre Five Year Strategic Plan 2014 – 2019.” We know that good bereavement and pre-bereavement care, including for children and young people, is essential in providing high quality palliative and end of life care. Fylde and Wyre CCG are publishing this RFI in order to better understand the bereavement and pre-bereavement service that are available for people locally, regionally, and nationally.
