Work description
Buckinghamshire Council is seeking to commission with an experienced and qualified provider for the co-ordination and delivery of a Buckinghamshire Accommodation Based Housing Support Service in Aylesbury. The Service will be delivered from a 36-bed hostel, Bearbrook Place, Oxford Road, Aylesbury, HP19 8RN for vulnerable single Buckinghamshire residents between the ages of 16 to 55 who are homeless or at risk of homelessness. Over a time limited period, the aim is to provide a supportive, person centred and empowering service to enable Service Users to gain the skills and support required to live independently, move-on to long-term housing solutions and sustain tenancies. The successful provider will be expected to work in partnership with health and social care services and the voluntary and community sector to promote personal independence, improved health and wellbeing and to prevent homelessness and reduce factors leading to repeated homelessness.
