AHS5129 - Providence House

Description

The London Borough of Tower Hamlets is seeking expressions of interest from interested organisations for the delivery of a package of Supported Accommodation Services for Vulnerable Adults with support needs at Providence House. The service is to deliver a person centred, personalised programme of support lasting between 3- 15 months to vulnerable adults which is reablement focused and to develop the skills and confidence to achieve optimum independence level and move on into appropriate long-term accommodation. The provider will be expected to achieve the outcomes and requirements of the specification and the Council will monitor compliance with the targets. It is expected that services will be subject to review, change and innovation over the course of its delivery.
