Lot 2: The Family Nurse Partnership (FNP) To provide this evidence-based licensed service, led by Family Nurses, working with first time young mothers(aged 19 and under), delivering the healthy child programme and providing additional support up to the child's second birthday. FNP will provide a specialised service to these mothers and support an integrated approach,working with other services in order to meet the needs of young children and their families, ensure smooth transition to other services, and deliver improved outcomes.
