## Description

Framework for the provision of Keeping Well Services Services that are being commissioned under this Framework are: Lots 1 to 5 - Care & Support at Home \(Daytime\) • Lot 1 - Sunderland East • Lot 2 - Sunderland West • Lot 3 - Coalfields • Lot 4 - Washington • Lot 5 - Sunderland North Lot 6 - Care and Support in the Home: Overnight Care Lot 7 - Live in Care Service Lot 8 - Carer Support Service \(Carer Relief\) Lot 9 - Support Services \(Ad-Hoc\) Care and Support in the Home The framework will be awarded as follows: Lots 1 to 5 - single contractor for each geographical locality area lot as stated above\. All framework orders will be directly placed with the contracted provider\. Lot 6 - single contractor lot covering all locality areas citywide\. All framework orders will be directly placed with contracted provider\. Lot 7 - single contractor lot covering all locality areas citywide\. All framework orders will be directly placed with contracted provider\. Lot 8 - single contractor lot covering all locality areas citywide\. All framework orders will be directly placed with contracted provider\. Lot 9 - multiple contractor lot covering all locality areas citywide by further competition or direct award
