1415-877 Community Based Enablement Service for Vulnerable Adults

People are living longer, but often without the support of an extended family. Our ageing population is placing greater demands on acute health services, and on community-based health and social care services. Prevention and early intervention services are a key element in the Council?s response to managing this challenge. The Council and Islington Clinical Commissioning Group (ICCG) already provide a mainstream re-ablement service to people coming out of hospital which is aimed at reducing reliance on on-going support from statutory services. The Community Based Enablement Service complements this provision, forming a part of the re-ablement pathway for vulnerable adults in Islington.
