## Description

People are living longer, but often without the support of an extended family\. Our ageing population is placing greater demands on acute health services, and on community-based health and social care services\. Prevention and early intervention services are a key element in the Council?s response to managing this challenge\. The Council and Islington Clinical Commissioning Group \(ICCG\) already provide a mainstream re-ablement service to people coming out of hospital which is aimed at reducing reliance on on-going support from statutory services\. The Community Based Enablement Service complements this provision, forming a part of the re-ablement pathway for vulnerable adults in Islington\.
