Work description
Provision for carers to have an assessment of their own needs, allowing them to discuss with a practitioner their caring role and how it affects their wellbeing, their feelings and choices about caring, what help they need to support them as a carer of a child/ young person with additional needs and / or disabilities, and to find out what help and support may be available.
