## Description

Provision of social prescribing service supporting people on the Sutton Health and Care Proactive caseload\. This service will provide personalised information, advice and support to people, and signpost or refer them to appropriate activities and services to meet their needs and preferences\. The service will develop relationships with and knowledge of local statutory and non-statutory organisations, groups and services to support the effective operation of the service\.
