## Description

This is the first annual reopening of the Home Care Framework\. This tender is split into the following lots: Lot A - Adults aged over 18 who meet the Care Act eligibility criteria for funded support, including all those who live within a housing with care environment\. Lot B - Adults aged over 18 who meet the Care Act eligibility criteria for funded support who have a learning disability and either lives within their own home or a supported living environment\. Lot C - Adults aged over 18 and children who meet the criteria for NHS funded care \(Continuing Health Care for adults and Continuing Care for Children\) Lot D - Children who meet the eligibility for funded support \(excluding those who sit within lot C\) Please see the Southampton e-portal for more information: https://supplysouthampton\.esourcingportal\.com/index\.cfm
