## Description

NHS Medway Clinical Commissioning Group and Medway Council \(the Contracting Authorities\) are seeking to procure a high quality, effective and outcome focused service that will improve health and wellbeing outcomes for local people and offer good value for money\. Commissioners want to pilot a new service model to support individuals living with long term health conditions to improve their skills, knowledge and confidence to manage issues impacting on their long term condition/s more effectively\. The pilot will take place initially in one Local Care Team locality \(Rochester\)\.
