To deliver the provision of the Sunderland Health Champions Network \. Provider will work in partnership with the Council and the Sunderland Live Life Well Service to create and develop a network that supports the Sunderland Health Champion Programme and promotes Making Every Contact Counts and the self-care agenda\.
