## Description

he overarching aim of this service is to support people to live independently in their own homes as long as possible\. This will be achieved through the following objectives: • Deliver Wirral Independence Service including the Telecare Monitoring and Response service and Falls Prevention Service • Provision of a Community Equipment Service \(CES\) for adults and children within the geographical boundary of Wirral Borough Council including those who live out of Borough and are registered with a Wirral GP\. The service includes\. • Pressure care, • Continuing Health Care \(CHC\), • Adult Visual Impairment & Hearing Impairment equipment, • Telecare and any future innovations \(equipment, installation, maintenance, monitoring, decontamination and repair and recycling\)\. • Mobility aids, • Moving and Handling, • Bariatric, • Schools Specialist Equipment, • Paediatrics, • Nebulizers • Toileting • Specialist non catalogue items • Minor Adaptations • Carers Emergency Response Cards Incorporating: • an online IT requisition\. • monitoring performance dashboard and reporting system, • Community based Retail Outlet/ Demonstration facility \(Centre for independent living\) • online self-help/assessment system and to include purchase and recycling opportunity • suitably qualified, knowledgeable, and experienced staff to support those funding their own care and to provide professional assessment and oversight within the service
