Whole Systems Integrated Care Services on behalf of NHS Harrow CCG

Provision of Enhanced Practice Nursing services and community based, multi-disciplinary healthcare for patients aged 65 and over within primary care networks in Harrow. The service will support improvements in health and wellbeing, enable patients to remain in their place of residence and will be flexible and adaptable, collaborating with other services and transitioning into integrated care teams.
