The buyer is seeking a contractor to support the system for a review of intermediate care processes, including pathways and practise in implementing discharge to assess (D2A), across Trafford, with special consideration for community bed-based provisions at Ascot House. Additional information: The contracting authority considers that this contract may be suitable for economic operators that are small or medium enterprises (SMEs).
