Description
The Public Health Department of The Royal Borough of Kensington and Chelsea, and the City of Westminster (the 'Authorities') have commissioned a Provider to deliver an Integrated Healthy Lifestyle Service ('the Service'). The aim of the service is to reshape the provision of adult healthy lifestyles services to ensure services meet the needs of residents in achieving Public Health Outcomes. The Provider shall meet the objectives of the Service which are: - reduce Cardiovascular Disease (CVD), causes of premature mortality and other health-related risk factors which contribute to health inequalities; - reduce overall mortality attributable to CVD especially among people living in more deprived areas; - reduce mortality from CVD and other risk factors in people under 75 years old; - increase the number of years lived without disability attributable to CVD and/or other risk factors; and - reduce lifestyle related cancers. In addition to the above objectives the Service shall deliver the following elements: - a face to face service working with the most at risk populations across the Authorities area. The face to face element of the Service shall: - Follow EAST design principles (Easy, Accessible, Sociable and Timely) - Be easy for residents to find - Be easy for referrers to find and refer into - Be innovative and flexible to the needs of Residents - support and update the One You Westminster and One You Kensington and Chelsea Digital Platforms to ensure residents have access to the most up to date national, and local, information, health campaigns, health apps and advice; - provide a telephone line which will field enquiries from local residents around Healthy Lifestyles and signpost to relevant services; - deliver training to specified organisations and front line workers; and - support GP and pharmacy engagement in the delivery of the objectives and administer financial transactions No other purchasers are named on the Framework Agreement, other than those already named in the Framework Agreement. The contract value covers the value of the framework awarded by Westminster City Council and the two call-offs by Westminster City Council and the Royal Borough of Kensington & Chelsea. Option for the contracting authority at its absolute discretion to extend the period of the framework agreement from three (3) years by a period of up to two (2) years in aggregate.
