Co-commissioned Complex Navigator

Description

A jointly commissioned complex needs navigator role between the London Borough of Hackney and the Probation service via an invitation to tender process to support clients who are considered at high risk of harm, and with complex and multiple support needs. This client cohort is likely to be managed at MAPPA levels 2 or 3, and may be part of the Integrated Offender Management (IOM) caseload. The navigator will work closely with the Benefits and Housing Needs Service and Probation Service.
