Description
NHS North West London ("NHS NW London") Integrated Care Board ("ICB") has secured a range of external resources and support towards Hypertension case finding and management. Cardiovascular disease is one of the two biggest contributors to the burden of disease in NWL's population. NWL's approach to hypertension currently focuses on managing hypertension in those already identified as hypertensive. There is a gap in identifying, engaging with and managing those with unidentified hypertension. Like all ICS, North West London has been set four core objectives: 1. To improve outcomes in population health and health care; 2. To tackle inequalities in outcomes, experience and access; 3. To enhance productivity and value for money; and 4. To help the NHS to support broader social and economic development. Deliverables from this work includes: 1. Identification of a cohort of individuals with undiagnosed hypertension in the exemplar boroughs within NWL; 2. The engagement of those identified individuals with local health and care services; 3. Testing of new sustainable working for GPs by utilising digital interventions and new workforce model to provide medical care for this cohort; and 4. Validation of a scalable methodology ahead of expansion across other NWL boroughs, with an agreed high-level roadmap to roll out to next 2-3 priority areas. During the project, The Boston Consulting Group UK LLP will deliver: • An analysis of population segments with lower-than-expected hypertension prevalence; • Identification of the characteristics of high-risk individuals within those segments; • Implementation of community specific approaches to engage with the target population. These will be delivered by a range of health and care professionals, and could include: o Digital tools in partnership with Microsoft (e.g. chatbot, remote blood pressure measurement and monitoring, virtual consultation) o Enhanced utilisation of other health care professionals (e.g. Pharmacist first approach) o Telephone callouts/texts • Development and application of behavioural modification approaches (e.g. nudge techniques) based on national and international examples; and • Analysis of financial implications including ROI, cost-benefit analysis and system-savings phased over time. The procurement for the Provision of PHM Hypertension was a direct award under the RM6187 Management Consultancy Framework Three (MCF3) and there was no requirement to issue a Tender Notice as per PCR 2015. The Authority used the ATAMIS Sourcing Portal at https://health-family.force.com/s/Welcome ("the e-sourcing Portal") to award.
