Description

Hull City Council is seeking a suitably qualified and experienced provider to deliver a Community Outreach programme, which aims to increase the uptake of NHS Health Checks across Hull. The service will deliver across a range of settings and specifically support an improvement of the uptake of NHS Health Checks among individuals at increased risk of developing cardiovascular disease. The contract will commence on 1 October 2018 and will run for 18 months, ending 31 March 2020, with an option to extend for up to 2 x 12 month periods. The contract will be a single lot; • A Community Outreach Programme, which will target known health inequalities in communities with respect to identifying individuals at an earlier stage of vascular change. This service will deliver NHS Health Check assessments in a range of settings (including workplaces and community venues for people who may not attend their GP Practice). In addition to delivering these services, we are seeking a provider who is able to deliver social value benefits for the City. These services are being tendered using an Open tender Process. Maximum budget available is £45,000 p.a. Any bid which exceeds this maximum budget will be eliminated from further process. TUPE will apply to this contract.
